My Wellbeing Check-In – What This Worksheet Is For
This worksheet provides a straightforward, private way to check in with how you are really doing, rather than automatically saying “I’m fine” and carrying on. It encourages you to notice your emotions, thoughts, energy, sleep and current pressures before difficulties become harder to manage.
What to do
Take a few quiet minutes and work through each question. You do not need to write long answers. A few words can be enough.
Think about how you are feeling, what has been occupying your headspace, your energy and sleep, and anything that has felt particularly difficult recently. Give yourself space to notice the positives too by identifying what has been going well.
Then consider what you need more of right now. That might be rest, connection, practical help, exercise, time alone, reassurance, better boundaries, a conversation with someone you trust or professional support.
How it can help
Regular check-ins can make it easier to recognise changes in your wellbeing before reaching the point of feeling completely overwhelmed. You may begin to notice patterns—for example, becoming more irritable when exhausted, withdrawing when under pressure, struggling to sleep when something is occupying your thoughts, or finding it harder to cope when you have had little time for yourself.
It can also help put feelings into words. You do not have to solve everything you identify; sometimes recognising what is happening is an important first step.
The final question—“One thing I can do for myself today is…”—encourages you to choose one realistic action rather than trying to change everything at once.
