What to do:
Hormonal changes can sometimes coincide with changes in mood, sleep, energy, concentration, memory and physical sensations. This worksheet gives you somewhere to record what you have personally noticed.

Think about changes in your body first, then consider mood, energy, sleep and concentration. You do not need to decide what is causing them.

If useful, complete the worksheet more than once so you can begin to notice whether any patterns emerge over time.

Consider what appears to make things easier or more difficult. This might include stress, sleep, routines, physical activity, food, workload or other demands.

Use the section about healthcare professionals to record anything you would like to ask or discuss at an appointment. Having your observations written down can make it easier to explain what you have been experiencing.
