A goal can be motivating, but it can also feel heavy when it is too broad, too rigid or disconnected from the realities of everyday life. My SMART Goal Plan offers a gentler way to turn an intention into a clear and workable next step.
The worksheet explores five parts of a supportive goal: Specific; Meaningful and Measurable; Achievable; Resourced and Relevant; and Time-Framed. This adapted wording keeps the familiar SMART structure while making room for health, energy, responsibilities, access needs and support.
Use the page for a personal, therapeutic, educational or wellbeing goal. You do not have to fill every space in one sitting. Begin with what matters, choose the smallest useful action, and return to review what you have learned. Changing a plan is not failure—it is part of making the goal fit your life.
