GOAL ACTION PLAN
Turn a goal into steps that feel manageable, meaningful and flexible. This practical worksheet helps you decide what matters to you, describe what progress could look like, break a larger goal into smaller actions and plan for the support or adjustments that may help.
The three-page guide includes space to name your goal in your own words, choose a first action, map out the next steps, identify possible barriers and create a backup plan. It also offers a gentle review so you can notice what is working and change the plan without treating a pause or setback as failure.
You can complete it in writing, talk through it with someone, use voice notes or drawings, or ask a trusted person to scribe. Use only the parts that are helpful. Goals can relate to home, work, study, health, relationships or personal interests, and the pace can change with your circumstances.
