A Lapse Doesn't Mean Failure
A lapse can bring disappointment, shame or the feeling that all previous progress has been lost. It has not. One event does not erase the work, learning or choices that came before it.
This worksheet treats a lapse as information rather than a verdict. It invites you to look at what happened, what you needed, whether there were warning signs and what might help next time.
Try to write with the same fairness you would offer somebody you cared about. Recovery and change are rarely perfectly straight lines. What matters now is what you can learn and what your next safer step could be.
