PREPARING FOR A MENTAL HEALTH APPOINTMENT
Appointments can feel much harder when you are already anxious, overwhelmed or trying to explain something that is difficult to put into words. This three-page workbook gives you somewhere to gather the important information before you go and to record what happens next.
Use it to note what has been happening, symptoms or changes you want to mention, how things are affecting everyday life, medication or treatment information, questions you want to ask, and any communication needs or adjustments that would help. The final page helps you record what was agreed, referrals, treatment changes and follow-up.
You do not need to complete every box. Take the pages with you and let them do some of the remembering for you.
If appointments make your mind go blank, this is for you. Write things down while you have the space to think. You do not have to explain everything perfectly, and you do not have to rely on memory when you are under pressure.
