HEALTH ANXIETY — THOUGHT RECORD SHEET
Notice the worry. Understand the cycle. Find a more balanced response.
Health anxiety can make a body sensation feel impossible to ignore.
You notice something. Your mind moves quickly towards a frightening explanation. Anxiety rises. You check your body, search symptoms online, ask for reassurance or seek confirmation that everything is okay.
The relief may help for a short while — until another doubt appears.
The Health Anxiety Thought Record Sheet is designed to help you slow that cycle down without dismissing genuine health concerns.
Inside the full guide you will find:
• an explanation of the health-anxiety cycle
• a full printable health-anxiety thought record
• trigger and body-sensation prompts
• feared health thoughts and belief ratings
• anxiety-intensity tracking
• checking, reassurance and online-searching prompts
• evidence and alternative-perspective questions
• guidance for finding a proportionate health response
• a balanced-thought exercise
• a checking and reassurance pattern worksheet
• a small behaviour experiment
• a Quick Health Anxiety Record
• guidance on when the worksheet itself may be becoming reassurance
• ways to recognise progress
• a personal message from me
• an important medical safety note
This is not about convincing yourself that every symptom is harmless.
New, severe, persistent, worsening or concerning symptoms may need appropriate medical assessment.
Instead, the worksheet helps you separate what you have actually noticed from what anxiety is predicting.
It also helps you recognise when checking, Googling, body monitoring or reassurance seeking is providing only temporary relief and keeping the cycle going.
The aim is a proportionate response: take genuine health concerns seriously, follow appropriate medical advice, and learn to recognise when anxiety is demanding impossible certainty.
Small changes matter.
One less search.
One delayed check.
One moment of returning your attention to everyday life.
