OCD OBSESSIONS & COMPULSIONS THOUGHT RECORD
Notice thoughts. Understand patterns. Choose a different response.
Intrusive thoughts, images, sensations, urges and doubts can feel extremely important when OCD attaches fear and uncertainty to them.
You may then feel driven to check, analyse, wash, repeat, seek reassurance, avoid, mentally review or perform another compulsion until things feel safe or certain.
The OCD Obsessions & Compulsions Thought Record is designed to help you notice that process without turning the worksheet into another form of reassurance.
Inside the full guide you will find:
• an explanation of obsessions and compulsions
• guidance on why an OCD thought record differs from a traditional thought-challenging worksheet
• a printable OCD thought record
• distress and uncertainty ratings
• prompts for visible and mental compulsions
• space to explore what happened after a ritual
• alternatives to reassurance and compulsive analysis
• a pattern-tracking worksheet
• process-focused questions for OCD
• information about CBT and Exposure and Response Prevention (ERP)
• guidance for recognising small steps of progress
• a personal message from me
The aim is not to prove that an obsession is false.
For somebody experiencing OCD, repeatedly gathering evidence, analysing a memory, checking feelings or searching for the perfect reassuring answer can become part of the compulsion.
Instead, this resource helps you ask different questions:
What is OCD asking me to solve?
What ritual am I being pulled towards?
What happens after I complete that ritual?
Can I allow some uncertainty to remain?
What would I choose to do if I did not have to solve this question first?
Progress might mean delaying a compulsion, reducing repetition, asking for reassurance less often, leaving something feeling “not quite right” or simply returning to your day while a doubt remains unanswered.
If completing the thought record starts to feel repetitive, compulsory or necessary to obtain certainty, step away from it. A helpful OCD worksheet should support you in reducing rituals rather than become another ritual itself.
